Alberta Application SAF1

Important information

  • Enter the information in the mandatory sections in the space provided on the application form. Do not leave any of the required areas blank

  • Put N/A for any question that does not apply to your circumstances

  • Do not complete sections 2,4,18 and 25(a) through to 32 and anywhere on the application where it is indicated that it is only for Agent/Broker and Company Use Only

  • All pages must be returned, this includes Pages 1 to 2 of Part 1 Government and Pages 1 to 3 of Part 2 Industry, for a total of 5 pages

  • If the mandatory fields are not fully completed and/or all pages are not returned, the application will need to be re-submitted, which will delay the process

Section 1

Section 1 is mandatory.

If there is more than one applicant, their names and primary address must be included in Section 12 Remarks or Section 33 Remarks continued. To avoid re-submitting the application, make sure you capture the entire address, including Unit number (if applicable), postal code, etc.

Section 3

Section 3 is mandatory.

If there are more than 4 vehicles to be insured, the vehicle details (same as what is required in Section 3) must be included in Section 12 Remarks or Section 33 Remarks continued. To avoid re-submitting the application make sure you capture the entire VIN number, which can be found in the vehicle registration or insurance documents, or on the vehicle dashboard.

You are not required to complete the Agent/Broker and Company Use Only section.

Section 4

You are not required to complete Section 4.

Section 5

Section 5 is mandatory.

You must list all drivers who will drive the vehicle(s) listed in Section 3 Described Automobiles. Enter the full legal name of the driver as shown on their driver’s license. Include the driver’s license number(s) and date(s) of birth.

If there are more than 4 drivers, the other drivers are to be listed in Section 12 Remarks or Section 33 Remarks continued.

For each driver, include the date they were first licensed in Canada or the United States and the class of license they currently carry. Leave the Grid Step blank. Indicate the percentage each listed driver uses the vehicle(s); percentage should total to 100% for each vehicle. For driver(s) who completed driver training, attach the certificate and include the date completed in Section 14 Additional Information for Drivers.

Section 6

You must provide details of All convictions arising from the Operation of Any Automobile during the past 4 years. Not including convictions for all the listed drivers will result in re-submitting the application.

If required, you can obtain an Alberta driver’s abstract via an Alberta registry agent, either in person or by providing a signed consent form. The abstract can also be ordered online at Alberta.ca

You must include details for all accidents or claims you and any listed driver received arising from the ownership or operation of any automobile during the past 6 years. To avoid re-submitting the application, make sure you capture all claims/accidents as requested in this section.

Section 7

Section 7 is mandatory.

You must answer the question asked in Section 7. If the answer is Yes, please state particulars in Section 24 Remarks or Section 33 Remarks continued.

Section 8

Section 8 is mandatory.

8(A): If you have been cancelled, declined, refused to be renewed or issued an insurance policy by any insurer, you must include the name of the insurer, reason and policy number.

8(B): You must enter your current (Aviva General) policy number and expiry date.

8(C): If you owe any money to another insurer related to Alberta auto policy, you must provide further details in Section 12 Remarks or Section 33 Remarks continued.

Section 9

Section 9 is mandatory.

If there are more than 4 vehicles, record the details for Sections 9A, 9B in Section 12 Remarks or Section 33 Remarks continued respectively.

9(A): Annual Distance – Enter the annual distance driven for each described vehicle

9(B): Vehicle Use – Indicate if the described vehicle(s) is used to commute. If yes, provide the one-way commuting distance.

9(C): Business Use – Record the % of annual KM driven for business use for each described vehicle(s). Enter zero if there is no business use.

9(D): Outside of Canada Use – Indicate if the described vehicle(s) is used outside of Canada. If yes, provide the details in Section 12 Remarks or Section 33 Remarks continued.

9(E): Modifications & Customizations – Indicate if the described vehicle(s) has any modifications or customizations that affect the original manufacturer’s design specifications or increase the value of the automobile. If yes, provide the details in Section 12 Remarks or Section 33 Remarks continued.

Section 10

Section 10 is mandatory.

10(A): Commercial use – If the described vehicle(s) is used for the purpose(s) indicated in the question, record the required details in the fields provided. If no, record ‘N/A’.

10(B): Transportation of Goods – If the described vehicle(s) is used for transportation of goods indicated in the question, record the required details in the fields provided. If no, record ‘N/A’.

Sections 11-12

Registered and Actual owner is mandatory. If you are not both the Registered and Actual owner of the described vehicle(s), you must provide the names of the Registered Owners and the Actual Owner.

Section 12 Remarks is mandatory if there is additional information from other section. Use Section 12 to bring clarity to any details captured in previous sections. If there’s not enough space, continue to record details in Section 33 Remarks continued.

Section 13

Section 13 is mandatory.

All applicants must sign the application; failure to do so will result in re-submitting it. Multiple applicants can sign on the same line. Digital signatures are acceptable. Note: you may have to download/print the completed application to add your signature.

Part 2

Part 2 of the application is optional.

Complete only those fields where you have additional information to provide, such as Driver Training details for listed drivers, anti-theft device installation on the described vehicle(s), etc.

You are not required to complete the following Part 2 sections:

  • Section 4: Optional Additional Coverage (Not required for S.A.F. 1)

  • Sections 28A, 28B, 29: Commercial Rated Vehicles

  • Section 29: Commercial Vehicle Use

  • Sections 30,31,32: Merchandise/materials and mounted machinery/equipment
